Waivers

After April 30th, we will contact all applicants who have been selected to participate in this year’s camp!

All participants and leaders are required to fill out a waiver and release form in order to attend camp.  Please contact us with any questions or concerns.

PLEASE DO NOT SEND US WAIVERS AND/OR $30 UNTIL YOU HAVE RECEIVED AN ACCEPTANCE EMAIL WITH THIS YEAR’S WAIVER PACKAGE.

Campers (Ages 18 & Under)

If you have already been accepted to camp, download the 2018 CampOUT! Camper Waiver and Release (18 & Under) and then please:

  1. Print
  2. Sign and/or complete the forms with a parent or legal guardian
  3. Return the signed pages of this package with a $30 cheque (if you did not pay online) by mail before May 30th to the following address:

Cabin Leaders & Leadership Team

If you have already been accepted to camp, download the 2018 CampOUT! Leadership Waiver and Release and then please:

  1. Print
  2. Read carefully and complete the forms
  3. We require your original signed document. Return the signed and completed pages of this form to CampOUT! in person at…

If you have not applied yet, apply before March 30th next year!

If you are on the waitlist, please be prepared to print, sign, and send in the forms if you are contacted when a space at camp has opened up.

Contact your local library or corner store if you don’t have access to a printer, scanner, or fax machine in your home, school, or workplace.
